You must have Adobe Reader installed on your computer to download and view our application forms. Please visit the Adobe website where you may download Adobe Reader for free.
Once you have Adobe Reader installed on your computer, simply click on one of the links below to download an application form.
Application Forms
Filling Out and Saving the Applications
These application forms have been designed to allow you to fill them out and save them on your computer. You can save the form by selecting File from the menu at the top of the screen and then selecting Save As from the drop down menu. A pop up box will appear that will allow you to select a location on your home computer to save the file in. The form does not need to be filled out all at once. You may fill out part of the form, save it and then finish and save it again at a later time.
Emailing the Applications
Once you have completed your application, you can email it to mkouri@kos.net. This link will open a new email message in your default email editor with the subject and body already filled in. You may change the subject or add more information in the email. You must attach the application form to the email as an attachment. Each email program may differ as to how a document is attached, but there will likely be a menu option called Insert and under it an option called File or Attachment. Once the application has been attached to the email and you have entered any additional information you want to include in the email, it is ready to be sent.
